Leading Age Washington presented the 2016 Excellence in Research & Education Award to CHCC’s CEO Patrick O’Neill at their annual conference in Spokane, WA.
Christian Health Care Center’s transition support team, made up of Faith Community Nurses specially trained in transition care, helps therapeutic rehabilitation patients make a successful and smooth transition home after their stay.
Care transitions at CHCC is a free service that is designed to help older adults get the follow-up care they need to remain at home and out of the hospital after surgery or an extended illness.
